How Can I Make An Appoinment?

Referrals and initial appointments are handled by calling 205-967-3277. Someone will schedule your appointment and answer any basic questions you may have about the clinic.

If you wish to use your insurance for reimbursement, be aware that most health insurance plans limit their coverage
for mental health services. Because of this we expect payment at the time of service and will be happy to file your
insurance for you for reimbursement. We do not participate in Medicaid, Medicare or any managed health care plans,
which require that you see a clinician from their local network. We encourage you to become familiar with the bene-
fits and exclusions of your particular plan, and to contact your insurance carrier’s claims representative question
about coverage in advance. You are primarily responsible for payment of all services provided to you or your
dependent minor.

Adolescents can only be examined with the authorization of their guardian or custodian. They youngsters have got to be properly informed about the reasons for their referral and nature of the clinical appointment in advance.

Please complete the registration and release forms (Forms), and review HIPAA Practices (Privacy) and the Welcome Our Practice office policies (Office policies) prior to the appointment. Bring this information with you to facilitate the
registration; otherwise, arrive 20 minutes ahead of your scheduled appointment to complete forms.

On the first visit we will review old records and history, meet with the patient and/or family for a psychiatric diagnostic examination, and make recommendations for further evaluation and treatment.

Payment is due on the day of your appointment. We accept cash, personal checks, Visa and MasterCard.
